Reporting/Recording Process for Incidents or Accidents

Purpose

The purpose of incident/accident reporting and investigations is to prevent a recurrence of the hazardous condition causing the event. It also ensures that we meet regulatory requirements.

Process

The University of Winnipeg’s Incident/Accident Report form must be completed for every incident or accident, even if there were no injuries sustained. If the injured person is a student or visitor the report form must still be completed. Any event that involves injury to a person or damage to property, or has the potential to do so, must be reported to the Lab Safety Officer (204.789.1439) within 24 hours of occurrence.

Injured employees must also complete a Workers’ Compensation Board (WCB) form, if appropriate. If you are not certain if you have WCB coverage, contact Mary Anne Walls (Manager, Campus Health & Wellness).

Information for Employees/Supervisors/Managers

If an employee suffers an injury while at work:

1. Report your injury to your supervisor immediately.

2. Seek medical attention or call 911 for emergencies.

3. The supervisor must contact the Lab Safety Officer (204.789.1439) or Campus Security (204.786.9272) to report the incident/accident.

  1. If it is a serious incident it must be reported to the Lab Safety Officer (204.789.1439) or call Workplace Safety and Health immediately (204.945.3446)
  2. A serious incident is any of the following:
    1. Death
    2. Collapse or structural failure of a building, tower, crane hoist, temporary construction support system. or excavation
    3. Explosion, fire, or flooding
    4. Fracture of a major bone
    5. Amputation, or cuts requiring hospitalization or time off work
    6. Loss of sight
    7. Third degree burns
    8. Unconsciousness resulting from concussion, electrical contact, asphyxiation
    9. Poisoning
    10. Any injury resulting in paralysis
    11. Any other injury likely to endanger life or cause permanent disability.

4. Complete an incident/accident report form & WCB form (if you’re covered) and fax (204.779.4624) or deliver to the Health & Safety Office (359 Young Street - 2Y12) within 24 hours. Where supervisors are unable to fully complete the form within 24 hours, the form, with as much information as is available at the time, should be submitted with the remaining information to follow later. The supervisor & employee review the recommendations of corrective action with a focus on prevention of recurrence.

5. During your absence from work, you must report regularly to your supervisor/manager, regarding your return to work date and/or changes in work schedule.

If no injury results & damage to equipment or property occurs:

1. Report the incident to your supervisor.

2. The supervisor must call Lab Safety Officer (204.789.1439) or Campus Security (204.786.9272) to report the incident/accident.

3. Complete an incident/accident report form and fax (204.779.4624) or deliver to the Health & Safety Office (359 Young Street - 2Y12) within 24 hours. Where supervisors are unable to fully complete the form within 24 hours, the form, with as much information as is available at the time, should be sent to the Safety Office with the remaining information to follow later.

4. The supervisor & employee review the recommendations of corrective action with a focus on prevention of recurrence.

Information for Students

1. Seek medical attention or call 911 for emergencies

2. Call Campus Security 204.786.9272 immediately 

3. Report the accident to a faculty or staff member 

4. Complete an incident/accident report form and submit to Health & Safety Office or Campus Security within 24 hours. Include statements obtained from witnesses

Responsibilities:

Supervisors/Managers

1. Immediately call the Lab Safety Officer (204.789.1439) or Campus Security Campus Security (204.786.9272) to report the incident/accident

2. Meet with employee to discuss the incident/accident causes

3. Investigate all incidents/accidents

4. Implement effective and practical action to prevent a recurrence of the hazardous condition. May require short-term corrective action measures to control immediate hazard, and long-term measures to effect change

5. Review the incident /accident form

6. Complete the supervisors/managers portion (and the WCB Employer’s Initial Report of Injury form for eligible employees if injury sustained, if necessary)

Employees

1. Immediately report your injury to your supervisor or designate.

2. Complete the incident/accident report.

3. Cooperate in the investigation.

4. If you suffer an injury while at work, complete the WCB Worker’s Initial Report of Injury form if you are one of the covered employee.

Joint Workplace Health and Safety Committee

1. Participate in the investigation of all accidents that require a employee to be hospitalized for a period of 24 hours or more and dangerous occurrences.

2. Review and analyze incident/accident reports and make recommendations.

3. Participate in Accident Investigation training.
