Claim Forms

The following claim forms are submitted directly to Manitoba Blue Cross:


The following claim form is submitted to Human Resources:

2012 Claim Deadline: Claim forms must be received by Human Resources no later than Friday, December 7, 2012.*

*Due to the taxable nature of the program, current year claims must be submitted for reimbursement in time to be processed for the final pay of the year.

Note: If you are making a claim for a Duckworth Centre membership that is being paid by payroll deduction, do not submit the claim form until the total amount of the deductions have reached the claim amount.
