Accidental Death & Dismemberment
The Accidental Death & Dismemberment (A.D.& D.) plan covers all eligible employees. The plan provides coverage in the event of death or injury from a work-related accident and is insured through SSQ Financial Group. The plan has been in effect since January 1, 1989.

Effective Date of Coverage
Coverage becomes effective on the date you commence regular employment. If you are not actively at work on the date your Accidental Death and Dismemberment coverage would normally become effective, coverage will commence on your return to work.
Premiums
The cost of this coverage is paid entirely by the University.
Benefits
The Principal Sum under this insurance is two times your basic annual salary (excluding stipends and overtime), rounded up to the nearest thousand. The minimum amount of coverage is $50,000, and the maximum amount of coverage is $100,000 per covered employee.
When injury results in any of the following losses within 365 days after the date of the accident the Insurer will pay for:

The maximum benefit for all losses resulting from the same accident cannot exceed the Principal Sum with the exception of Paralysis, where the maximum benefit cannot exceed two times the Principal Sum.
Beneficiary Designation
The benefit payable in the event of Loss of Life is payable to the beneficiary you have designated for your Group Term Life Insurance. All other benefits are payable to you. If you are no longer eligible for the Group Term Life Insurance, you will be asked to make a beneficiary designation that is specific to the Accidental Death and Dismemberment plan.
Rehabilitation
In the event that you sustain an injury for which a claim would become payable under the Accidental Death & Dismemberment plan and, as a result of the injury, require special training for the purpose of being qualified to engage in a special occupation in which you would not have engaged except for the injury, the Insurer will pay reasonable and necessary expenses incurred by you for the special training, up to a maximum of $10,000. The training must take place within three years of injury.
Business Travel Coverage
You are covered under the Business Travel rider of this policy if you are:
a) Traveling directly to or coming from a meeting either regular or special;
b) Participating as a delegate member of the University at any convention relating to the Universitys affairs at the expense of the University;
c) Traveling on the business and/or in the interest and/or at the expense of the University.
Coverage under section a) above, applies when you leave your residence to go directly to the meeting place, along a normal route, without delay or stop over.
Coverage under sections b) and c) above, applies only during the course of the trip. Such a trip shall be deemed to commence when you leave your residence or place of regular employment for the purpose of going on the trip, whichever occurs last, and will continue until you return to your residence or place of regular employment, whichever occurs first.
Exclusions
No benefits would be payable under this plan if death or injury should occur due to any of the following reasons:
- Suicide or self-destruction
- War (declared or undeclared)
- While on active full-time service in the armed forces
- Riding as a passenger or otherwise in an aircraft not possessing a current and valid certificate of airworthiness
Termination of Coverage:
Coverage will terminate on the earliest of the following dates:
- Termination of employment, retirement or death
Conversion
Due to the work-related nature of the Accidental Death and Dismemberment coverage, this plan may not be converted to individual insurance.
